Please fill out the form below and a registration packet will be sent via inter-mail to your office location. Directions of how to fill out the form can be found here. After you have filled out the registration packet you can send it back via inter-mail to the USofCC office (600 S. Michigan, Rm 1305B) or Flor Carabez (600 S Michigan, Room 303).

* Filling out the form below does not mean you are signed up. You must fill out the registration packet and submit it in order to officially sign up for the union.  
* after 90 days, unit members will be automatically enrolled as Agency payers and a bill will be sent to their home. Failure to pay the invoice can lead to termination.
